The physicians at the Community Family Health Center are committed to providing you and your family with care throughout your lifetime. Your family physician is the one who gets to know you well and understands your healthcare needs. All our physicians are on the medical staff of Howard Regional Health System. This means that sophisticated specialty services and inpatient and outpatient procedures can be scheduled quickly, attended by your CFHC physician who knows your medical history.

For your convenience we have Online Forms that you can fill out in advance when planning a visit to the Community Family Health Center. By printing out and completing the forms in advance you can be certain you'll arrive at the center with the necessary information in hand.
